I get this issue sporadically as well (and have been for like a year), but I can't seem to reproduce what causes it. I do know for a fact I have seen colored flags visible on the minimap that I had not had vision on.

When there is too much packet loss, source engine will resend a full update of all entities. Because of this you get an updated state of the control point.
